Attribute in User object unavailable on Request form

I have an object that I created on the User object that I want to add to a form to allow the user to fill in/select

I linked User to Request, but only a limited # of attributes are showing in the User collection, & this attribute is one of the ones missing.  Is there something I should look for, or can I do this a different way?
